Answer: The qualification review shall be based on the written certificate issued.

The first one should be blank and signed and stamped by the unit that issued the certificate. The name needs to be signed. If it is left blank, you can sign the name of the person in charge of the unit. Don't leave it empty.

Don’t worry about the second and third blanks. The examination agency review agency will sign and seal them one by one.
