The guardian of the newborn must take a copy of the newborn's household registration book and a one-inch photo of the newborn to the neighborhood committee of the community where the household registration is located to fill out the "Urban Resident Basic Medical Insurance Registration Form", which will be signed and stamped by the handling personnel. The insurance registration form must be sent to the Residential Section of the Municipal Medical Insurance Center to make a social security card and pay medical premiums.
