1. Applicant's resident identity card and household registration book 12. If the applicant has cancelled the account number 12, submit the household registration record certificate issued by the public security organ where the original household registration is located.
2. Proof of kinship issued by the personnel department of the applicant's unit. If there is no personnel department in the applicant's unit, a certificate of kinship shall be issued by the location of the applicant's file or the superior competent department; If the applicant has no work unit, the certificate of kinship between himself and the related person shall be issued by his street or office (township and town people's government).
3. If you entrust others to handle notarization on your behalf, you shall submit the power of attorney and your resident ID card.
4. The related party shall submit the resident ID card, household registration book 12, passport, pass and other documents that can prove his identity.
5. Other documents and certification materials that the notary believes should be submitted. Of course, different notary offices may have different requirements for materials and certificates, so it is recommended to consult the local municipal notary office first.
The proof of mother-child relationship refers to the relationship of rights and obligations between mother and child in law. Mother and son are the closest lineal blood relatives and an important part of family relationship. If it is necessary to prove kinship with parents who are not in the same household 12 (if they are not in the same household 12), they can only bring their original ID card, birth certificate, parents' ID card and other materials to the local notary office for notarization of relatives. At this stage, the public security department will no longer issue proof of kinship. Parents and citizens sharing the same household with me can directly provide the original household registration book 12 to confirm their kinship. For parents who are not in the same household (12), the household registration department has the right not to issue a household registration certificate because the kinship can not be found. Therefore, citizens can only bring their parents' ID cards, their household registration ID cards (12), ID cards and certificates to the local notary office for notarization of kinship.
