General rules for checking and judging unqualified work tickets
1. Work tickets that meet any of the following conditions shall be counted as incorrect or unqualified work tickets:
< p>2. The work tickets are not numbered or have duplicate numbers or wrong numbers, are not signed or stamped as required, are graffitied or dirty on the work tickets, or are lost or damaged.3. The work location or work tasks are unclear or incorrect. When filling in the work tasks, the equipment has double numbers but is not filled in. The equipment numbers are altered or the handwriting is unclear and difficult to read.
4. Safety measures are not consistent with the work tasks, or safety measures are expanded without reason, safety measures are incomplete but not supplemented, and the live parts reserved at the work site are not filled in, incomplete, or incorrect.
5. Failure to fill in the exact installation location, group number and number of the grounding wire,
6. Failure to indicate the location where the signboard is installed or the name of the signboard is wrong or the installation mistake.
7. If the operator fails to fill in the relevant content in the electrical ticket as required on the face of the ticket or the filling is unqualified, he should fill in the work ticket again according to regulations but fails to fill it out.
8. The person in charge of the work is changed or the work is postponed without completing the procedures according to the regulations. The issuer of the work ticket, the person in charge of the work, and the work permit holder do not meet the regulations or exceed the scope of approval. The wrong type of work ticket is issued.
9. Failure to perform the closing procedures as required or failing to inspect and accept the work-ending ticket or dismantle safety measures.
10. Unsafe phenomena such as exceptions, failures and attempts occur during execution.
11. Work tickets filled in, issued and approved without authorization or exceeding authority.
12. The surface of all work tickets is not clean and the handwriting cannot be read; the electronic version of the work ticket is altered; the handwritten version of the work ticket is altered and the handwriting is unclear.
13. There are typos in key words in the work ticket (including operating verbs, professional terms, names and numbers of equipment, work locations, etc.).
14. Before the maintenance work started, the work permitter and the person in charge of the work did not go to the site to check the safety measures together and agreed to sign.
15. Failure to strictly follow the work ticket process for filling in, issuing, permitting, extending, discontinuing, and terminating procedures.
16. Changing personnel, changing work tasks, changing the scope of work, moving the work site, expanding or reducing the scope of work without complying with regulations.
17. The work permitter and the person in charge of the work go through the work ticket termination procedures without *** jointly conducting a comprehensive inspection and acceptance of the site.
18. The completed work tickets are not stamped with the corresponding seal or the closing procedures are incomplete.
