2. Interim Provisions on the Qualification System of Licensed Pharmacists
Chapter I General Provisions
Article 1 In order to ensure the implementation of the Qualification System of Licensed Pharmacists and strengthen the registration management of licensed pharmacists, these Measures are formulated in accordance with the Interim Provisions on the Qualification System of Licensed Pharmacists jointly issued by the Ministry of Personnel and National Medical Products Administration.

Chapter II Application for Registration
Article 6 After obtaining the Licensed Pharmacist Qualification Certificate, the personnel of a pharmaceutical production, trading and use unit may apply to the licensed pharmacist registration agency in the area where the practicing unit is located for registration.
Article 7 A person who applies for the registration of a licensed pharmacist must meet the following conditions:
(1) Obtaining the Qualification Certificate of Licensed Pharmacist; ?
(2) Abide by the law and professional ethics; ?
(3) Being in good health and able to persist in working as a licensed pharmacist; ?
(4) with the consent of the practicing unit. ?
article 8 registration shall not be granted under any of the following circumstances:
(1) Not having full capacity for civil conduct; ?
(2) Due to criminal punishment, it is less than 2 Shaanxi from the date of completion of punishment to the date of application for registration; ?
(3) Those who have been punished for canceling the qualification of licensed pharmacists are less than two in Shaanxi;
(4) other circumstances in which it is not suitable to engage in licensed pharmacist business as stipulated by the state. ?
Article 9 A person who applies for registration for the first time must fill in the Application Form for First Registration of Licensed Pharmacists and submit the following materials:
(1) Qualification Certificate of Licensed Pharmacists;
(2) a copy of the identity certificate;
(3) 5 recent one-inch bareheaded frontal half-length photos;
(4) a physical examination form issued by a hospital at or above the county level within 6 months;
(5) certificate of practicing unit;
(6) a copy of the certificate of the legal opening of the business unit.
article 1 the period of validity for the registration of licensed pharmacists is three Shaanxi. The holder must apply to the original licensed pharmacist registration institution for re-registration three months before the expiration of the validity period. Those who fail to go through the re-registration formalities after the time limit, their Licensed Pharmacist Registration Certificate will automatically become invalid, and they can no longer practice as licensed pharmacists.
Article 11 To apply for re-registration, an application form for re-registration of licensed pharmacists shall be filled out and the following materials shall be submitted:
(1) Licensed Pharmacist Qualification Certificate and Licensed Pharmacist Registration Certificate;
(2) the examination materials of the practicing units;
(3) Registration Certificate of Continuing Education for Licensed Pharmacists;
(4) a physical examination form issued by a hospital at or above the county level within 6 months. ?
Article 12 Anyone who has obtained the Qualification Certificate of Licensed Pharmacist and completed the credits of continuing education according to regulations may retain the qualification of licensed pharmacist. To apply for registration after obtaining the Qualification Certificate of Licensed Pharmacist-Shaanxi, in addition to the provisions of Article 9, it is also necessary to submit the Registration Certificate of Licensed Pharmacist's Continuing Education containing the record of his participation in continuing education.
chapter iii registration and management?
Article 13 A licensed pharmacist registration institution shall register qualified pharmacists within 3 working days from the date of receiving the application; Those who do not meet the requirements will not be registered, and the applicant will be notified in writing and the reasons will be explained. ?
Article 14 A licensed pharmacist registration institution shall register according to the professional category indicated in the Licensed Pharmacist Qualification Certificate of the applicant for registration.
Article 15 When a licensed pharmacist registration institution handles registration, it shall affix a special seal for registration in the registration column of the Licensed Pharmacist Qualification Certificate and issue the Licensed Pharmacist Registration Certificate uniformly printed by National Medical Products Administration.
Article 16 If a licensed pharmacist changes his practice unit or scope in the same practice area, he/she must go through the registration formalities of change at the original licensed pharmacist registration institution, fill in the Registration Form of Change of Licensed Pharmacist, and submit the following materials:
(1) Licensed Pharmacist Qualification Certificate and Licensed Pharmacist Registration Certificate;
(2) a copy of the certificate of the new practice unit's legal opening; If a licensed pharmacist changes his practice area, he shall go through the registration formalities at the original licensed pharmacist registration institution, fill in the "Registration Form of Licensed Pharmacist Change", and re-apply for registration at the licensed pharmacist registration institution in the new practice area. The new licensed pharmacist registration institution shall withdraw the original Licensed Pharmacist Registration Certificate and issue a new Licensed Pharmacist Registration Certificate when handling the registration procedures.
Article 17 After registration, a licensed pharmacist shall be deregistered in any of the following circumstances:
(1) Dead or declared missing; ?
(2) being subject to criminal punishment; ?
(3) The Licensed Pharmacist Qualification Certificate has been revoked;
(4) being subject to administrative sanction of dismissal; ?
(5) unable to engage in licensed pharmacist business due to health or other reasons. The cancellation of registration procedures shall be applied to the registration institution by the unit where the licensed pharmacist works within 3 working days, and the "Registration Form for Cancellation of Registration of Licensed Pharmacists" shall be filled in. After verification, the licensed pharmacist registration institution shall cancel the registration and recover the Licensed Pharmacist Registration Certificate.
Article 18 A licensed pharmacist registration institution shall report the registration status to National Medical Products Administration for the record every Shaanxi, and make regular announcements.
Article 19 National Medical Products Administration has the right to order the licensed pharmacist registration agency to review and correct any licensed pharmacist who fails to meet the prescribed conditions.
Article 2 A party who disagrees with the refusal or cancellation of registration may apply for administrative reconsideration or bring a lawsuit to a people's court according to law.
Article 21 Anyone who registers by fraudulent means, transfers, borrows or forges a Licensed Pharmacist Qualification Certificate, a Licensed Pharmacist Registration Certificate and a Licensed Pharmacist Continuing Education Registration Certificate shall, once found, have his registration certificate confiscated and his registration cancelled. Those who constitute a crime shall be investigated for criminal responsibility according to law.
Article 22 Any staff member of a licensed pharmacist registration institution who neglects his duty, abuses his power or engages in malpractices for personal gain in the registration work shall be given administrative sanctions by his unit according to relevant regulations; If a crime is constituted, criminal responsibility shall be investigated according to law.
Chapter IV Supplementary Provisions
Article 23 A person who holds a Licensed Pharmacist Qualification Certificate is not registered, does not have the status of a licensed pharmacist, and may not engage in licensed pharmacist business activities, and the certificates related to licensed pharmacist business issued by him are null and void.
Article 24 A practicing entity refers to a legal entity that produces, deals in and uses drugs.
Article 25 National Medical Products Administration shall be responsible for the interpretation of these Measures.

Special note:
Registration of licensed pharmacists for permanent residents in Hong Kong and Macao
The State Food and Drug Administration has issued a notice on relevant matters, which clearly states that from October 1, 29 in Shaanxi, the food and drug supervision departments of all provinces (autonomous regions and municipalities) will accept applications for registered licensed pharmacists submitted by permanent residents in Hong Kong and Macao who have obtained the Qualification Certificate for Licensed Pharmacists in the Mainland, and follow the Interim Measures on Amending and Printing the Registration Management of Licensed Pharmacists. When applying for practicing registration in the Mainland, permanent residents of Hong Kong and Macao who have obtained the Qualification Certificate for Licensed Pharmacists in the Mainland must submit the Employment Certificate for Persons from Taiwan, Hong Kong and Macao, the original of the Hong Kong Pharmacist License or the Macao Pharmacist License, and submit a copy at the same time, and the practicing unit should be consistent with the employer indicated in the Employment Certificate for Persons from Taiwan, Hong Kong and Macao.
