Measurement calibration certificate is the form in which the measurement unit expresses the parameters of the measuring instrument.
The certificate and test report at least include the following information:
1. Certificate and report name , for example: the words "Calibration Certificate";
2. Laboratory name and mailing address;
3. The unique number, page number and total number of pages of the certificate and report, There is a clear end mark;
4. Name and address of the client;
5. Description, status and clear identification of the sample;
6. Sample Date of receipt, date of calibration/testing;
7. Basis and method of calibration/testing (technical specifications and document code);
8. Standard device used for calibration/testing Traceability, such as: calibration certificate number and validity period of the standard instrument, etc.;
9. Environmental conditions for calibration/testing, such as: temperature, humidity, etc.;
10. Calibration/ Testing location;
11. For items that allow exceptions to deviate from standards and procedures, the deviation content should be noted in the calibration/test results column;
12. Verification and calibration of report results , the signatures of the testing personnel, verification personnel and the authorized signatory who issued the certificate/report;
13. The calibration results given in the calibration certificate should include the measurement uncertainty;
14 .When applicable and necessary, the certificate/report can provide opinions and explanations;
15. A statement that it is only valid for samples and that the certificate and report may not be partially copied without approval.
