Two. Submission of materials
To apply for labor ability appraisal, an application form for labor ability appraisal shall be filled in and the following materials shall be submitted:
(a) the original and a copy of the "work-related injury determination decision";
(2) Complete medical records such as valid diagnosis certificates, copies or duplicates of inspection reports that meet the relevant provisions of medical institution medical record management;
(three) the original and photocopy of the valid identity certificate such as the resident ID card or social security card of the injured employee;
(four) other materials as prescribed by the labor ability appraisal committee.

After receiving the application for labor ability appraisal, the labor ability appraisal committee shall conduct a timely review; If the materials provided by the applicant are incomplete, the applicant shall be informed in writing of all the materials that need to be supplemented within five working days from the date of receiving the application for labor ability appraisal.
If the materials provided by the applicant are complete, the labor ability appraisal committee shall organize the appraisal in time.

According to the degree of injury, the labor ability appraisal committee randomly selects 3 to 5 experts from the medical and health expert database to form an expert group for appraisal.

The labor ability appraisal committee shall notify the injured workers in advance of the time, place and materials. Workers with work-related injuries shall participate in on-site appraisal according to the time and place notified. For workers with work-related injuries who are inconvenient to move, the labor ability appraisal committee may organize experts to conduct on-site labor ability appraisal.
The expert group put forward the appraisal opinions according to the injury of the injured workers, combined with medical diagnosis and according to the national standard "Grade Appraisal of Injured Workers and Occupational Diseases". Experts participating in the appraisal shall sign their opinions and signatures.
When experts disagree, the expert opinion of the expert group shall be determined according to the principle that the minority is subordinate to the majority.

The labor ability appraisal committee makes the conclusion of labor ability appraisal according to the expert group's appraisal opinions, and issues the conclusion of labor ability appraisal.
The labor ability appraisal committee shall make a conclusion of labor ability appraisal within 60 days from the date of receiving the application for labor ability appraisal. If the injury is complex and involves multiple medical and health professions, the time limit for making the conclusion of labor ability appraisal may be extended by 30 days.
Seven, service appraisal conclusion
The labor ability appraisal committee shall, within 20 days from the date of making the appraisal conclusion, timely deliver the appraisal conclusion to the injured workers and their employers, and send a copy to the social insurance agency.
Eight. reappraise
Article 16 If an injured worker or his/her employer refuses to accept the conclusion of the initial appraisal, he/she may apply to the labor ability appraisal committee of the province, autonomous region or municipality directly under the Central Government for re-appraisal within 15 days from the date of receiving the appraisal conclusion.
IX. Review and appraisal
After the conclusion of labor ability appraisal is made 1 year, if the injured workers, employers or social insurance agencies think that the disability situation has changed, they can apply to the municipal labor ability appraisal Committee with districts for labor ability review and appraisal.
If you are not satisfied with the conclusion of the re-examination appraisal, you may apply to the provincial labor ability appraisal Committee for re-examination appraisal.
