Article 2 The attached Liquor Circulation List shall be printed by the Provincial Department of Commerce in a uniform number according to the style required by the Ministry of Commerce, and the self-made list shall be invalid.

Article 3 The Supplementary List of Liquor Circulation shall be distributed according to the principle of "whoever registers shall distribute it", and the municipal, county and district commercial bureaus shall be responsible for distributing the Supplementary List of Liquor Circulation to the liquor operators registered in their own organs.

Second, the scope of use

Article 4 The liquor circulation schedule is an important document of the liquor circulation traceability system, which is only used in the liquor circulation process and may not be used for other internal management of the enterprise.

Article 5 The sales departments of liquor production enterprises (including wholesale and retail enterprises, hereinafter referred to as liquor commodity suppliers) and liquor wholesale operators must issue a Liquor Circulation List when selling.

Article 6 Liquor retailers (including service providers such as hotels, restaurants, bars, nightclubs, individual industrial and commercial households, hereinafter referred to as buyers of liquor, the same below) shall take the initiative to obtain the accompanying list of liquor circulation when purchasing liquor.

Article 7 When transporting, storing and allocating alcoholic commodities, both the supplier and the demander shall ensure the consistency of one commodity (batch) and one single commodity, and the single commodity shall go with the goods. If there is a single commodity, it must be available. However, alcoholic commodities stored in the enterprise by the sales department of an alcoholic production enterprise are excluded.

Eighth beer production and sales do not implement the "alcohol circulation voucher" system. Medicinal liquor, health wine and cooking wine are not subject to the filing and registration system and the accompanying list of liquor circulation.

Ninth "liquor circulation list" in triplicate, the first copy for the stub; The second copy shall be kept by the sales unit; The third is to go with the goods.

Third, the application method

Article 10 Every six months (May 65438+February), the County Bureau of Commerce applies to the Municipal Bureau of Commerce for the attached list of liquor circulation, the purchasing book of Yunnan Province's attached list of liquor circulation and the allocation list.

Materials to be submitted when applying for the County Bureau of Commerce:

1. When applying for the first time, it is required to submit the Application Form for the Schedule of Liquor Circulation List and the Semi-annual Demand Calculation Form signed by the unit leader and sealed by the unit;

2. When applying again, it is required to submit the Application Form for the Schedule of Alcohol Circulation, the Registration Form for the Distribution of the Schedule of Alcohol Circulation (Summary), the Half-year Demand Calculation Form, the Use Stubs of the Schedule of Alcohol Circulation and the Transfer Stubs.

The application procedure of the County Bureau of Commerce:

Article 12 A supplier of alcoholic commodities shall apply to the administrative department of alcoholic circulation in the place where the enterprise is registered for the List of Alcoholic Circulation Accessories and the Purchase Book of Alcoholic Circulation Accessories in Yunnan Province.

Materials to be submitted when applying for liquor supplier:

The first application must be submitted:

1. Application Form for Schedule of Liquor Circulation Enterprises;

2. A copy of the registration form for liquor circulation;

3. Product inspection report;

4. A copy of the business license;

5. The production enterprise shall provide the production license;

6. Letter of introduction from the unit and valid certificate of the agent.

To apply again, you need to submit:

Application procedures for liquor suppliers:

After accepting the application, the liquor circulation management department must, without special reasons, issue the List of Liquor Circulation Accessories and the Purchase Book of Yunnan Liquor Circulation Accessories to the applicant within two working days, and issue the allocation list.

Four. Attachment management

Article 13 The administrative department in charge of liquor circulation in counties and districts shall arrange special personnel to apply for and keep the liquor circulation schedule to the superior administrative department, be responsible for relevant examination, calculation, distribution, registration, stub recovery and storage, and be responsible for the summary and reporting of relevant information. Liquor operators must designate special persons to be responsible for collecting and keeping the attached liquor circulation list.

Article 14 The administrative department in charge of liquor circulation at the county and district level must establish the receiving and dispatching account of the list of liquor circulation accessories (the registration form for the issuance of liquor circulation accessories), and carefully register the name of the receiving enterprise, the registration number for filing, the number of recipients, the starting and ending numbers of the list of accessories, the date of receipt, the signature of the recipient, the signature of the issuer, the production license number of the production enterprise, the main business brands, stub recovery and other contents. The supplier of alcoholic commodities must establish the Schedule of Yunnan Liquor Circulation Purchase Book.

Fifteenth liquor suppliers must truthfully fill out the "Liquor Circulation Attached Sheet", requiring clear handwriting and seal. Buyers of alcoholic commodities must truthfully check the attached alcoholic circulation list with the relevant documents of the seller and the purchased alcoholic commodities, so that the documents are consistent and the single goods are consistent. The attachment is invalid without the seller's seal. Suppliers and buyers of alcoholic commodities must be responsible for the alcoholic commodities they supply and buy.

Article 16 The Liquor Circulation List shall not be reused, lent, opened on behalf of others, forged or traded.

Article 17 If the List of Accessories for Liquor Circulation is lost or damaged, the relevant information and the starting and ending numbers of the lost or damaged documents must be immediately reported to the liquor circulation management department that issued the List of Accessories for Liquor Circulation.

The county liquor circulation management department shall timely record relevant information and report to the municipal liquor circulation management department. After receiving the report of loss or damage, the county-level liquor circulation management department shall promptly urge the unit that lost or damaged the List of Accessories for Liquor Circulation to publish a notice of loss or damage to the List of Accessories for Liquor Circulation in the municipal news media within 10 working days, and the expenses for the notice shall be borne by the unit that lost or damaged it.

If the attached liquor circulation list cannot be used for special reasons, it shall be returned to the issuing unit in time.

Article 18. For alcoholic commodities that do not implement the "additional list of alcoholic beverages" system, once verified, the alcoholic beverage circulation management department shall give a warning according to Article 28 of the Measures for the Administration of Alcoholic Beverage Circulation, order it to make corrections, and make an announcement to the public; Refuses to correct, depending on the seriousness of the case, impose a fine of 5000 yuan, and announced to the public.

Article 19 This system shall come into force on June 1 2008.
