1. Students and parents' information: generally including parents and students, names, gender, contact information, school name, grade and class, etc. Just fill in the corresponding position and check the selected category in the box/.

2. Place of residence before returning to school: here, including the current address, and other places that I have been to before leaving my current address, such as my hometown address after returning to my hometown.

3. Health status: fill in the physical health status, fever and other symptoms at a specific time (such as before returning to school 14 days).

4. Have you ever been in contact with high-risk personnel for epidemic prevention and control, living at home or in isolation? If you have contacted, check None or No; if you have contacted, select Yes or, and add the contact date and specific description.

5. Promise and sign, the situation is true: finally, the guardian can confirm the authenticity and accuracy of the health card information and sign it. Some need to be audited by school personnel, and this is done by the school.
