(2) Show the original personal ID card and submit two copies. If it is a second-generation resident ID card, a copy of the back of the ID card is required;
(3) Submit the Social Insurance Certificate for Employees in * * or the Shenzhen Labor Security Card;
(4) One copy of the employee's personal payment list (print the payment records from the first month to the last month of the employee's insurance, which can be printed in the social security management office of the district or in the business center of the personal service center);
(5) acceptance letter from the social security agency in the transfer place (the acceptance letter shall indicate the account name, full name of the bank and account number);
(6) The entrusted agent for re-insurance matters shall also provide a power of attorney issued by the client (the power of attorney shall be signed and fingerprinted by the client, stamped with the official seal or personnel seal of the client's unit, or provide a notarized power of attorney), and two originals and photocopies of the agent's ID card; If there is a balance in the medical insurance or housing provident fund account, a copy of the client's bankbook or bank card should be provided (note: bankbooks and bank cards that have been opened for less than one month will not be accepted), and an application for transfer should be filled out in duplicate.
note: if the personal account of the employee who is transferred to insurance still has the personal account balances of medical insurance and housing provident fund, the employee must also go to the bank window to get the bank debit card (the personal account balances of medical insurance and housing provident fund are stored in the card) when the employee comes to handle the transfer business himself; When the agent comes to handle the re-insurance business, the agent provides a copy of the client's bank passbook or bank card and fills in the transfer application form in duplicate, and then receives another settlement document in the financial window, and the social security agency is responsible for remitting the balance of personal accounts of medical insurance and housing provident fund to the above account.
