1. application form (mht) document: in duplicate, only with official seal, in duplicate; Please print according to the screenshot below. (Headers and footers are not allowed)
2. Copy of business license: 2 copies, printed in black and white on A4 paper, with official seal in the blank; (The latest annual inspection stamp 20 10 is required, and the copy should be clear. )
3. Name confirmation form: signature (anyone who can confirm the name)+official seal.
If the information is wrong and needs to be changed, please contact the staff of our center first, and don't change it without authorization.
Four. GPL description: in duplicate; Stamp with official seal;
Five, the early development guarantee: two copies; Fill in the name of the developer (2-5 people), print it, and then sign it with the official seal; Attach a copy of the developer's ID card (front and back, original size) and sign it (in duplicate);
6. Certificate of legal person of superior unit: in duplicate, with information of superior unit filled in. After printing, both parties shall affix their seals and the superior legal representative shall sign it.
7. Copy of the business license of the superior unit: in duplicate; Stamp with official seal. (stamped with 20 10 annual inspection seal)
Matters needing attention in printing mht files are as follows:
1. Open the file, and then click File and Page Setup in the menu bar.
2. Clear all information in the header and footer.
3. Print it in duplicate after processing, and affix the official seal to the applicant's signature in the figure below. Remember: the applicant does not need to sign after affixing the official seal.
