Instructions for filling in the form
1. When applying to establish a society, you must fill in the "Student Association Preparation Application Form" carefully and truthfully;
2. Each item must be filled in, and if there is no answer, fill in "none";
3. When applying for establishment, the society should also submit the following information: draft articles of association; basic information on the sponsor and proposed person in charge Introduction, identity certificate; basic situation report of the instructor, identity certificate; other relevant materials that need to be submitted. Please print (copy) the following materials on A4 paper and attach them to this application as an attachment. The original identity certificate will be returned to the applicant after verification;
4. This application form is made in triplicate. Please fill it out with a blue or black pen or ink pen. One copy should be retained by the Student Association, one copy should be retained by the Youth League Committee of the college, and one copy should be retained by the Youth League Committee of the college. The community exists on its own.

Remarks: 1. The preparation for the establishment of student clubs is carried out by the Federation of Student Clubs of the College and the Human Resources Department Mainly responsible for the final interpretation of this system belongs to the Federation of Student Associations;
2. The basic process of preparing and establishing a student association:
① This should be filled in carefully by the sponsor who is preparing to establish the association. Submit this document together with any relevant information in any form;
② Compare all the information submitted by the society and track and verify the authenticity of the information;
③All members of the Human Resources Department
④ Fill in the meeting report and submit it to the instructor of the Student Association Federation of the college;
⑤ Obtain the report from the college Decision of student club advisor.
