If you don't want to and it's inconvenient (teachers generally don't speak English), use written form.
In other words, you need to print it out.
Of course, if you can use PEF editor, you can also use it to fill out forms.
But note that any place that needs the teacher's signature must be handwritten, including the date of signature.
Generally, teachers should sign more than a dozen, depending on the number of schools you apply for.
After all the forms are filled out, just put your school envelope with the letter of recommendation in Chinese+English.
