1. HPV vaccine and other vaccines cannot be vaccinated at the same time. The interval between the two vaccines should be at least 14 days.

2. If the appointment is successful, those who are in poor health during the scheduled vaccination period, are preparing for pregnancy or during pregnancy and lactation, or have received other types of HPV vaccines and the interval between vaccinations with other vaccines is less than 14 days, Vaccination will not be given and the quota will not be reserved. It is recommended that you do not apply if the above conditions exist.

3. Please be sure to fill in the registration information carefully. Those who have successfully made an appointment can only get the vaccination quota for themselves. They must be vaccinated on time after the appointment is successful and cannot be transferred. Those who do not come for vaccination on time will be deemed to have voluntarily given up.

4. During vaccination, the appointment information, personal name, ID card and SMS information on the mobile phone will be checked. All of the above are indispensable. If any conditions are found to be inconsistent, vaccination will not be given.

5. The winning SMS will be sent directly to the user as soon as the lottery is over. Please bring your ID card and medical insurance card when getting vaccinated, and strictly abide by the hospital's epidemic prevention and control requirements. Currently, people in high-risk areas will not participate in the lottery, and winning numbers will not be retained.

6. In order to ensure the consistency and fairness of the reservation process, only APP reservations are accepted. Other reservation methods such as telephone reservations, WeChat reservations, and on-site reservations are not accepted. The reservation quota is limited, so please understand.

6. Reservation steps

1. Open the Zhejiang Office APP, switch to Shengzhou City, Shaoxing City, search for the vaccination application, enter the vaccine online reservation platform, and go to the adult vaccine page on the homepage. Click on the HPV vaccine module.

2. The lottery instructions will pop up when entering the interface. Applicants must read the lottery instructions carefully and click I understand.

3. In the upper left corner, select Shaoxing City, the nine-price HPV vaccine and the lottery activities of the vaccination clinic of Lushan Street Community Health Service Center in Shengzhou City, and click to apply.

4. Check the event details carefully and confirm that you meet the event requirements. Click Apply Now to verify your information. If it is correct, click Confirm Application, which means the application is successful.

5. After the application is successful, the user can see the applied activities in the application list, and can cancel the appointment according to the actual situation.

Warm reminder: An ID card can only make a successful reservation once, and reservations can only be made with my real-name account. Reservation is not possible and cannot be transferred once the reservation is made. If you need to make a new appointment, please cancel your application first.

6. After the lottery is over, the successful SMS will be sent to the user as soon as possible.
