1. The crew applying for the health certificate is over 18 years old, and the validity period of the health certificate shall not exceed 2 years;
2. If the age of the crew applying for the health certificate is less than 18, the validity period of the health certificate shall not exceed 1 year.
A health certificate contains the following basic contents:
1. Name, gender, date of birth, nationality, department on board, signature and photo of the holder;
2. Certificate number;
3. Applicable provisions of relevant international conventions;
4. Declaration of recognized medical practitioners;
5. Date of issuance and expiration date of validity;
6. Name of authorized organ;
7. Name of issuing institution and signature of chief physician;
8. Other contents that need to be explained.
An applicant for a health certificate shall meet the following conditions:
1, at least 16 years old, but not more than 65 years old;
2. Hold valid identity documents;
3, in line with the requirements of the crew health inspection standards.
The applicant for health certificate shall submit the following materials:
1, valid ID;
2. Crew Health Checklist;
3. Photographs that meet the requirements of the maritime administrative agency.
To sum up, the physical examination institution shall, according to the application of the crew, conduct a physical examination of the crew; The chief physician shall make a decision on whether to issue a health certificate within 2 working days from the end of the crew health examination. If it meets the requirements, it shall issue a health certificate; For those who do not meet the requirements, issue the Crew Health Checklist, and keep a copy of the Crew Health Checklist.
Legal basis:
Article 6 of the Measures for the Administration of Health Certificates for Seafarers in People's Republic of China (PRC) shall be valid for no more than 2 years. If the crew applying for a health certificate is under the age of 18, the validity period of the health certificate shall not exceed 1 year.
Article 7 crew members of seagoing vessels shall apply for health certificates from health inspection institutions recognized by maritime administrative agencies.
