Domestic consignee registration certificate and import license for imported solid waste that can be used as raw materials; apply for a domestic consignee registration certificate for imported solid waste that can be used as raw materials at the local inspection and quarantine bureau, and import Apply for a license at the environmental protection department.
The following basic materials are required to apply for a domestic consignee registration certificate:
(1) "Application for Registration of Domestic Consignee of Imported Waste Raw Materials": *** 3 pages, should be Provide the original copy.
(2) Industrial and commercial business license (copy)
(3) Organization code certificate (copy)
(4) "Foreign Trade Operator" Registration Form and other import and export qualification license documents (copies)
(5) Organizational structure, department and job responsibilities of the consignee
(6) Quality control of imported waste raw materials , employee training, disposal of unqualified goods and other quality management system documents or quality system certification certificates (color copies)
(7) Those engaged in importing waste raw materials (type, quantity and environmental quality) in the past two years Relevant instructions
(8) The "Registration Certificate for Overseas Suppliers of Imported Waste Raw Materials" (photocopy) of the corresponding overseas supplier. The copy must be stamped
The seal of the overseas supplier and the signature of the legal person. (9) The import entrustment documents of the domestic processing and utilization enterprises that we represent, the organization code certificate (copy) and the "Registration Form for Imported Solid Waste Utilization Units that Can be Used as Raw Materials"
The following basic materials are required for an import license:
1. Written application report.
2. Application form for import license of solid waste that can be used as raw materials. (See Appendix 1)
3. Environmental protection reports of enterprises that process and utilize imported solid waste that can be used as raw materials. (See Attachment 2)
4. A copy of the valid annual inspection of the legal person business license. (Should or include a description of the solid waste business scope)
5. A copy of the valid organization code certificate for annual inspection.
6. A copy of the valid annual general taxpayer qualification certificate (or the national tax registration certificate stamped with the words "VAT general taxpayer").
7. Color photos and text descriptions of factory processing sites (including factory entrance and brand, factory overview, raw material and finished product storage sites, processing and utilization sites), facilities, equipment, pollution prevention and control facilities and equipment. (A text description is written under each photo to explain the scene or function shown in the photo)
8. Copies of the construction project environmental impact report (form) and its approval documents.
9. A copy of the project completion environmental protection acceptance approval document.
10. Reference format of the record book on the processing and utilization of imported solid waste. (See Appendix 7)
11. Environmental monitoring plan and emergency monitoring plan system.
12. For self-monitoring, provide monitoring qualification certificate and certificate of employment certificate; maintenance and calibration plan of monitoring instruments, and calibration record results. (If the enterprise conducts self-monitoring, it must be provided, but if it is not self-monitored, it does not need to be provided.)
13. If monitoring is entrusted, the entrustment contract and the monitoring qualification certificate of the entrusted monitoring agency should be provided. (Required for entrusted monitoring, not required for non-entrusted monitoring)
14. Management systems and measures to prevent the import of solid waste that does not meet national environmental protection control standards.
15. Rules and regulations regarding environmental protection job responsibilities and assessment standards; environmental protection training materials for relevant staff and managers, etc.
16. Copies of clean production audit, evaluation and acceptance certificate documents. (Enterprises subject to mandatory cleaner production audit must provide this material, otherwise, they do not need to provide it)
17. Copy of the registration certificate of the domestic consignee of imported solid waste that can be used as raw materials .
18. A copy of the customs declaration registration certificate of the consignee and consignor of import and export goods.
19. Table on the supervision and management of solid waste that can be used as raw materials for (fill in the name of the company) that applies for automatic import license. (See Appendix 4)
