8. Filling out and issuing work tickets (including work task sheets).
(1) The work ticket should be filled in and signed with a black or blue pen or ballpoint pen in duplicate. The content should be correct, the filling should be clear, and no alteration should be allowed. If there are any errors or omissions that need to be corrected, standard symbols should be used and the handwriting should be clear. Keywords and words are not allowed to be modified. Keywords and words are determined by the enterprise based on actual conditions.
(2) Computer-generated or printed work tickets should use a unified ticket format, be verified by the issuer of the work ticket, and must be signed manually or electronically before execution.
One copy of the work ticket should be kept at the work place and collected by the person in charge of the work; the other copy should be collected by the work permit holder and handed over according to the value. The work permit holder shall record the work ticket number, work tasks, permit and end time in the register.
(3) In a work ticket, the issuer of the work ticket, the person in charge of the work and the work permit holder shall not serve as concurrently with each other.
(4) The work ticket should be filled in by the person in charge of the work or by the issuer of the work ticket.
(5) The work ticket is issued by the equipment operation and management unit, or by the repair, testing and infrastructure unit that has been reviewed and approved by the equipment operation and management unit. The list of work ticket issuers and work leaders of the repair, testing and infrastructure units should be submitted to the relevant equipment operation and management units for filing in advance. When the issuer of the work ticket deems it necessary, he or she may use a general work ticket or a sub-work ticket and issue them at the same time. The filling and licensing of general work tickets and sub-work tickets and other relevant regulations shall be implemented after approval by the leader in charge of production (chief engineer) of the unit.
(6) In contracting and subcontracting projects, work tickets can be "double issued". 9. Use of work tickets.
(1) A work leader can only issue one work ticket, and the work location listed on the work ticket is limited to the scope of the safety measures on the work ticket. All safety measures in the work ticket should be completed at once before starting work.
One work ticket is allowed to be used for equipment maintenance work of the same type on the same equipment system in sequence.
(2) When multiple teams are working simultaneously within the same equipment system and within the same safety measures, a work ticket can be issued to the general person in charge, but the main work content must be filled in in detail. In the column of working class members, only the person in charge of each class should be filled in, and it is not necessary to fill in the list of all staff members.
(3) When the same type of maintenance work is performed on different equipment systems in different locations in sequence, if all safety measures can be completed at one time before the work starts, one work ticket can be used.
(4) Before the end of the maintenance work, if the following situations are encountered, the work ticket should be re-issued and the work permit procedures should be performed again:
1) Some maintenance equipment is added to the operation time;
2) The isolation method between maintenance and operating equipment must be changed or safety measures need to be changed or added;
3) The maintenance work cannot be completed after being postponed once and needs to be continued.
(5) When the work ticket is damaged and cannot be used anymore, a new work ticket should be replaced.
(6) The work ticket should generally be sent to the person in charge of the operation duty in advance one day before the start of work, and the temporary work permit should be given to the person in charge of the duty on the day of work. Work orders may be given to the work permittee on the day the work is performed.
(7) Before permitting to start work, the person in charge of duty shall arrange for on-duty personnel to implement the safety measures listed in the work ticket. Important measures (determined by the power plant) should be supervised and implemented by the person in charge of the shift or the main work types such as the driver, stoker, and central control supervisor. If power outage measures need to be carried out by electrical personnel on duty, the "power outage contact sheet" should be used.
(8) When it is necessary to change the members of the work team, the work should be approved by the person in charge of the work, and the new staff can only start working after a safety briefing.
(9) If it is necessary to change the person in charge of the work, the original issuer of the work ticket should agree and notify the work permit holder, and the work permit holder will record the change on the work ticket. The work leader of non-consecutive work is allowed to change once. The original and current work leaders should hand over work tasks and safety measures.
