Need to provide:
(1) the valid identity certificate and photocopy of the family (or client) of the deceased, and indicate the relationship with the deceased on the photocopy;
(2) A copy of the valid identity certificate of the deceased (babies and nameless corpses without household registration need not be provided); The doctor in charge of treatment issued a death certificate.
Non-hospitals (including homes, old-age care institutions and other places, except those where doctors have visited the site) issue certificates: the identity certificate of the deceased and the death fact certificate, and the local health center handles the death certificate (inference).
Relevant information includes:
1. Original certificate of neighborhood (village) committee;
2. The original and photocopy of the deceased's valid medical record, identity card or household registration book;
3. The original and photocopy of the ID card of the agent's family (or the client), and make a Death Certificate that is not issued repeatedly.
Second, the instructions for filling in the death certificate?
1. This certificate shall be filled in by the doctor in the hospital (clinic) or the inspector of the administrative and judicial organs immediately after death.
B, in the case of administrative or judicial inspection? The certificate under the column shall be signed and sealed by the institution and inspectors.
Please fill in all figures in Chinese capital letters to comply with the regulations (especially the date of birth and death).

5. The filling method of each column of this certificate is as follows:

(4) Fill in the detailed address of the deceased's residence.
(5) Fill in the date of birth of the deceased and calculate the age of the deceased in the last column ().
(6) Fill in the detailed time of death of the deceased in the column.
(7) Fill in the detailed location and place of death of the deceased in the column. If it is a hospital, tick the back, and so on.
(8) Fill in the column of death type of the deceased. If the deceased died of illness or natural causes, put a tick in the last □, and so on.
(9) Fill in the column of the deceased's detailed work (such as the names of institutions, schools, companies, factories, gardens, woodlands, etc.). ) and fill in what kind of industry (such as taxation, health, administration, commodity trading, planting rice and wheat manufacturing machinery, etc.). ) and what kind of work and position (such as business manager, section chief, typist, accountant, etc.). )
(10) Fill in the marital status of the deceased. If the deceased is unmarried, tick □ at the end, and so on.
(1 1) Please refer to the detailed classification table of 1975 (for doctors) to list the causes of death. Please also pay attention to the sequence of causes and the approximate period from onset to death. If the victim died of injury, please fill in the external cause of injury.
Please fill in the name of certificate, hospital (clinic) and business license.
G. The note number column of this certificate shall be filled in by the health unit personnel according to the provisions of the Operation Manual for Death Cause Statistics.
Note: Please bring this certificate to the household administration office for death registration within 30 days after the death occurs or is confirmed.
3. What are the laws related to death certificates?
According to the Law of People's Republic of China (PRC) on Medical Practitioners and the Regulations on the Administration of Medical Institutions, the hospital is only responsible for issuing the Medical Certificate of Death for residents who died in the process of diagnosis and treatment, and the following procedures must be strictly performed:
1. According to the national hospital work regulations and hospital work system, after the death of a patient, a death (autopsy) notice (signed by the family) shall be issued to the patient's family in time;
2, by the attending doctor (medical practitioners) to fill in the "death report card";
3. Send the death case report card to the hospital administrative hall, and the death medical certificate manager will check the patient's family, ID card and household registration book, then fill in the death medical certificate and affix the official seal of the unit.
4. The patient shall apply for a medical certificate of death within 3 days after death. The patient's family should properly keep the death certificate, keep one for themselves and lose it.
