Please note that the format is day/month/year, for example, 06/09/1989 (September 6, 1989).

4. Town and country of birth – Town of birth

Just fill in the Chinese pinyin, such as BEIJING (Beijing).

5. Nationality - Nationality

For China, write CHINA.

6. Occupation - Occupation

7. Contact address in the UK (in full) - Contact address in the UK

You can fill in the address of a relative in the UK or Full address details of the hotel where you are staying.

8. Passport no. - Passport number

The passport number is in the upper right corner of the page with the photo in the passport. Just copy it and don't fill it in wrongly.

9. Place of issue - Passport issuance place

Check where the passport was issued. For example, if the passport was issued in Beijing, write BEIJING, CHINA, and if the passport was issued in Guangzhou, write GUANGZHOU, CHINA.

10. Length of stay in UK - Length of stay in the UK

It can be within the validity period of the visa, such as 1 MONTH (1 month), or write down your visa number Length, such as UNTIL JAN, 1ST, 2016 (before January 1, 2016).

11. Port of last departure - Last departure point

Here you can fill in the departure point of the plane, such as PU DONG AIRPORT (Pudong Airport), and the transfer airport.

Must be signed on the passport Consistent Chinese Pinyin.

The remaining parts with a blue background will be filled in by UKBA staff, so just ignore them.
