Article 1 Party B meets the qualification conditions for home-based aged care services stipulated in the Implementation Plan for Socialized Home-based Aged Care in Jinshui District, and voluntarily applies for and agrees with Party A to arrange attendants to provide home-based aged care services. If Party B does not meet the qualification requirements, Party A will have the right to stop or refuse to provide services.
Article 2 The service object of home-based care for the aged is the elderly, and Party B shall consciously abide by it. In the course of service, if Party B requests or disguised requests the waiter to serve other personnel other than Party B, the waiter has the right to refuse or stop the service as the case may be.
Article 3 The specific work of service providers in providing home-based aged care services is as follows:
(a) to help the elderly cleaning, laundry, cooking, shopping and other basic living services;
(two) to assist the elderly to seek medical treatment, decoction, rehabilitation and other basic nursing services;
(three) to accompany the elderly to chat, walk, psychological counseling and other spiritual comfort services.
Article 4 Without the consent of Party A, Party B shall not increase the labor burden beyond the agreement in Article 3 or seriously inconsistent with the home-based aged care service, and Party A reserves the right to stop the service. If Party B is not satisfied with the service provided by the waiter, Party B has the right to apply for the replacement of the waiter.
Article 5 Service arrangement:
Party A will arrange home-based care workers with health certificates and graduation certificates to serve Party B according to the "five-fixed" method of "setting service targets, service items, service hours, service places and service personnel". If the service personnel arranged by Party A this time are not suitable for Party B's living environment, Party B may apply to Party A for replacement of service personnel.
Article 6 Service fee:
(1) According to the Application Registration Form for Home Care for the Aged in Jinshui District filled out by Party B, Party B meets the subsidy standard for home care for the aged in Jinshui District and belongs to the type of (a□ b□ c□), and can enjoy (1□ 2□ 3□) hours of home care service every day, and the required labor fee is subsidized by the government at 3.75 yuan per hour.
(2) Party B is obliged to fill in the Certificate of Home Care Service correctly on the last day of each natural month or the day when the service stops, and pay the service fee of 3.75 yuan/hour to the waiter on time according to the actual service time of the month. If Party B fails to pay the service fee to the waiter on time, Party A will stop the service, and Party B shall pay the waiter a late fee of 1% of the total service fee of the current month every day.
Article 7 Service adjustment:
(1) Both Party B and the waiter have the right to adjust the service. If you need to adjust the service for any reason, you must apply to Party A three days in advance.
(2) After receiving the service adjustment application from Party B or the waiter, Party A will stop the service according to the actual situation and send a new waiter to the door in time, but there is no guarantee that the service will not be interrupted during the service adjustment.
(3) After the service adjustment, this agreement is still valid.

(1) After the waiters take up their posts, Party B shall respect the waiters and treat them equally, and shall not discriminate or abuse them. If you are not satisfied with the waiter, you can directly contact Party A for personnel adjustment.
(2) If an emergency happens to Party B within the service period specified by the waiter, the waiter will contact the relevant guardians of Party B in time, and at the same time contact the corresponding social emergency agencies such as: 1 10, 120,19, etc.
(3) After the waiter arrives home, Party B has the obligation to explain in detail the matters that should be done and matters needing attention, and teach the waiter to use household appliances and electrical appliances, and put forward reasonable work requirements face to face, so that the waiter can carry out his work quickly and stably.
(4) Party A has clearly stipulated that waiters are not allowed to bring relatives and friends to Party B's home during the service period. In case of the above situation, Party B may contact Party A and deal with it immediately (unless invited by Party B). (5) Party A has explicitly prohibited waiters from borrowing money, lending things or entrusting things to Party B. In case of the above situation, Party B shall explicitly refuse, and Party A shall not be responsible for any disputes arising from private behavior.
(6) Flight attendants are not allowed to take personal leave for more than three times a month, and each time is not allowed to exceed three days. Party B is welcome to supervise. Party B will not pay the service fee during the vacation.
(7) Legal holidays (Tomb-Sweeping Day 1 day, May Day 1 day, Dragon Boat Festival 1 day, Mid-Autumn Festival 1 day, 1 month 13 days, New Year's Day/kloc). If Party B needs a waiter's statutory holiday service, and the waiter agrees, Party B shall pay 300% of the service fee according to the national regulations.
(VIII) Party B shall always take good care of cash and valuables. If Party B thinks that the property has been stolen because it cannot be found, it shall report the case to the public security organ in time and solve it through legal channels. Under no circumstances shall Party B frisk, rob, beat or intimidate the waiter.
(9) If the waiter causes accidents to himself or others during the service, Party B shall immediately notify Party A. ..
(X) Party A has the right to refuse Party B to transfer the waiter to a third party, and Party B may apply to Party A for adjustment.
(1 1) Party A shall not be responsible for the adverse consequences caused by Party B's common acute diseases such as heart, brain and blood vessels in the elderly.
(12) During the period when Party B is hospitalized due to illness or suffers from infectious diseases, his children shall fulfill the obligation to support and take care of the elderly, and Party A shall suspend the service and continue to arrange the service after Party B recovers.
Article 10 This Agreement is made in duplicate, with each party holding one copy. This agreement shall come into force from the date of signature by both parties until the conditions for dissolution and termination agreed in this agreement appear.
