Four. Procedure:

(1) The Provincial Communications Department, the Provincial Water Resources Department, the Provincial Bureau of Geology and Mineral Resources, the Provincial Bureau of Coal, the Provincial Yellow River Bureau, the Provincial Electric Power Group and the Provincial Metallurgical Corporation are responsible for organizing the declaration of safety production licenses for construction enterprises in this industry.

(2) The central construction enterprises in Shandong directly report to the Provincial Construction Management Bureau.

(three) the application for the safety production license of construction enterprises outside the above scope shall be organized and implemented by the municipal construction (construction) department in charge of the enterprise registration.

(4) After the above-mentioned provincial departments and local municipal construction (construction) departments have passed the examination, the enterprise application materials shall be submitted to the administrative licensing window of the Provincial Construction Management Bureau, which shall be transferred to the Quality and Safety Department.

(5) The Quality and Safety Department of the Provincial Construction Management Bureau conducts the safety production license audit, and the qualified enterprises report to the director in charge for examination and approval, and publish them in the form of documents one week after the publicity of Shandong Construction Information Network; If the examination fails, the application materials shall be returned, and the relevant competent department shall be notified in writing to explain the reasons. Municipal construction (construction) departments and relevant provincial departments will inform relevant enterprises to carry out rectification, and can declare again after rectification.

(a) the application for the safety production license of the construction enterprise

1, declaration form:

(1) Whether the "Basic Enterprise Information" and the following "Contents Required by Original and Copy" are consistent with the business license, the "Basic Enterprise Information" must be blank and signed by the legal representative himself. (The indicated enterprise address must be marked "Shandong Province", and the main person in charge must be the legal representative).

(2) The manager shall not concurrently serve as the deputy manager in charge of safety, and the deputy manager in charge of safety shall not concurrently serve as the head of the safety organization;

2. Application materials:

(1) Contents of responsibility system for production safety and rules and regulations for production safety at all levels and contents of documents and operating procedures;

(2) documents to ensure the investment in production safety (including the management measures or rules and regulations of the enterprise to ensure the investment in production safety, the annual investment plan of safety funds and its implementation);

(3) Documents on the establishment of safety production management institutions and full-time safety production management personnel (including the documents on the establishment of safety management institutions by enterprises, the job responsibilities of safety management institutions, the post-holding documents of the person in charge of safety institutions, and the detailed list of members of safety management institutions);

(4) The list and certificates (photocopies) of the principal persons in charge of the enterprise, project leaders and full-time safety production management personnel;

(five) the list of special operations personnel of this enterprise and their operation qualification certificates (photocopies);

(6) The annual safety training and education materials for the managers and operators of this enterprise (including enterprise training plans and training assessment records);

(seven) the relevant certificates of employees participating in industrial injury insurance and dangerous workers participating in accidental injury insurance on the construction site;

(8) Inspection certificate of construction hoisting machinery and equipment;

(9) Preventive measures against occupational hazards (corresponding preventive measures should be formulated according to the types of occupational diseases that may be caused by the operating characteristics of this enterprise);

(10) Preventive monitoring measures and emergency plans for sub-projects with high risks and parts and links prone to major accidents on the construction site (according to the business characteristics of this enterprise, the parts and links prone to accidents of sub-projects with high risks are listed in detail, with targeted and operable control measures and emergency plans);

(1 1) Emergency rescue plan for production safety accidents (based on the principle of effective rescue after accidents, a detailed list of rescue organization personnel, rescue equipment, equipment list and rescue drill records shall be listed);

(12) When binding, the application of color pages should be separated by twelve items in the catalogue. All materials should be numbered in sequence (including copies). Copies of social industrial injury insurance and accidental injury insurance sheets must be bound separately and attached to the application materials, except in the application materials.

(2) Extension declaration of safety production license

1, reapply for extension.

Construction enterprises belonging to the following scope shall re-examine their conditions for safe production:

(1) Within the validity period of the safety production license, a production safety accident occurs and is responsible for the accident;

(two) in the period of validity of the safety production license, the safety production license is temporarily withheld;

(3) During the validity period of the safety production license, he has been punished by the competent construction departments at all levels for more than 3 times (including 3 times), informed criticism or has a bad record of safety production integrity;

(four) the annual examination of safety production conditions and safety production evaluation are basically qualified and unqualified, and have been basically qualified after repeated rectification in the process of safety production license declaration;

(five) did not apply for an extension within the prescribed time;

(6) The safety production license supervision and management department considers it necessary to re-examine the enterprise safety production evaluation, project safety evaluation and daily supervision and management.

2. Extension of normal statement

During the validity period of the safety production license, a construction enterprise that strictly abides by the laws, regulations, rules and mandatory standards related to safety production and does not belong to the scope of review may stop the review with the consent of the supervision and administration department of safety production license. However, such enterprises still need to submit relevant application materials as required.

3, material requirements:

(1) Three months before the expiration of the safety production license, the enterprise shall apply to the Provincial Construction Management Bureau for extension.

(2) The enterprise that needs reexamination shall submit the following application materials: ① Application Form for Extension of Safety Production License of Construction Enterprise in triplicate (see Annex); (2) The original business license, qualification certificate, safety production license of the construction enterprise, and a copy of the "Class III personnel" assessment certificate of safety production (the "Class III personnel" assessment certificate should be within the validity period before the enterprise safety production license is extended) (the original should be submitted); ③ Relevant documents and materials specified in Item (3) to Item (5) and Item (7) to Item (13) of Article 7 of the Detailed Rules for the Administration of Safety Production License of Construction Enterprises in Shandong Province (Provisional).

(3) Enterprises that postpone the normal declaration shall submit the above materials.

In addition to the extension application form, the enterprise extension application materials (including copies of all original certificates and vouchers) shall be bound into volumes. When applying for extension, the enterprise needs to submit the originals of all certificates and vouchers. The enterprise shall be responsible for the authenticity of the substantive contents of the application materials.

(three) the declaration of the change or loss of the relevant contents of the safety production license.

1. If the name, address and legal representative of the construction enterprise are changed, a written application shall be submitted within 10 working days from the date of change of the industrial and commercial business license, and the original safety production license, the changed industrial and commercial business license, the certificate of change of the industrial and commercial business license and other relevant certification materials shall be presented to the municipal construction (construction) competent department or the relevant provincial department for the change. It shall be reported to the municipal construction (construction) competent department or the relevant provincial department for review, and the municipal construction (construction) competent department or the relevant provincial department shall fill in the "Verification of the consistency between the original and the photocopy, and agree to declare" and the opinions of the municipal construction (construction) competent department or the relevant provincial department, affix the official seal of the department, and submit it to the Provincial Construction Management Bureau for the change of the safety production license together with a copy of the change materials.

2. If a construction enterprise loses its safety production license, it shall hold the Report on Application for Reissuing and the Statement of Loss and Invalidation published in a comprehensive newspaper at or above the provincial level, fill in the Supplementary Application Form for Safety Production License of Construction Enterprises in Shandong Province, and submit an application for reissuing the safety production license to the municipal construction (construction) department or the relevant provincial department, which shall report to the Provincial Construction Management Bureau.
