Card Security Guarantee Claims Process

1. Card Security Guarantee Protection Rights Description for Loss of Personal Documents:

(1) Document loss protection covers 5 major categories of documents, namely: ID card; Passport; pass; driver's license; household registration book.

(2) Compensation is limited to the loss of the above documents. Renewal upon expiration or re-issuance if not lost is not covered by the insurance and cannot be compensated.

(3) The premium version pays a fixed amount of 200 yuan each time, and the annual compensation limit is 1,000 yuan.

(4) If multiple documents are lost at one time, compensation can only be paid once.

2. Claim materials for re-issuance of certificates

1. Please see attachment 1 for the claim application form (fill in, sign, provide account information, etc.). It must be printed out, filled in by hand and signed;

2. Please take a photo of the cardholder’s China Merchants Bank credit card with the card number;

3. Please take a photo of the cardholder’s identity certificate (front and back of the ID card (if the ID card is lost) , please choose the last two identity certificates), passport, household register (
