On the basis of confirming party member's identity and obtaining the above-mentioned certification materials, party member's current Party branch is responsible for filling out the "Volunteer Letter for Joining the Party". The process is as follows:

1. The cover of the Volunteer Letter for Joining the Party reads: "Due to the loss of the Volunteer Letter for Joining the Party, in? Month? On the same day, it will be reissued and stamped with the official seal of the party branch or unit.

2. Please fill in pages 1 to page 6 of the Volunteer Letter for Joining the Party. When filling in the form, it should be noted that the columns such as "personal experience", "reward or punishment", "major family members" and "major social relations" should reflect the situation when joining the party, and do not fill in what happened after joining the party. The date at the end of page 7 should be the date when he joined the Party, followed by the date of replacement, and the words "replacement date" should be written.

3. Paste the above-mentioned certification materials in the column "Problems to be explained to Party organizations" on page 7.

4. The columns of "Opinions of Party Introducers" and "Opinions of Party Organizations at Higher Levels Assigning Special Persons to Talk with Party Applicants" shall, in principle, be signed by the original party introducers and speakers, fill in the date of joining the Party, then fill in the date of appointment, and write the words "Fill in". No introducer or spokesman can be found, but the former party branch secretary can explain the situation.

5. Hold a branch meeting, make a resolution, and agree to reissue the "Volunteer Letter for Joining the Party". The Party branch committee should report the investigation and verification to all party member in detail, and form a branch resolution on this basis. It is stated in the column "The branch meeting accepts the applicant's resolution to prepare for party member" on page 9 of the application for joining the party, and the "resolution" should indicate the year, month and day when the preparatory period begins; The resolution is entitled "Resolution on Agreeing to the Comrade's Supplementary Volunteer Letter". The contents of the resolution should include: the investigation and verification of party member's identity, the number of official party member who should attend and actually enjoy the right to vote, the voting results (the number of pros and cons and abstaining votes in completing the volunteer letter) and the date of adoption of the resolution, which should be signed and sealed by the party branch secretary.

6. Hold a meeting of the Party General Branch Committee, make a resolution and agree to reissue the "Volunteer Letter for Joining the Party". Fill in the general party branch's opinions in the corresponding column of "General Party branch's examination (approval) opinions", and the content will be unified as "Agree to the branch resolution", which will be signed by the secretary of the general party branch and stamped with the official seal.

7. The Party Committee made a resolution to agree to reissue the volunteer letter for joining the Party, and in the corresponding column of the "Party Committee's examination and approval opinions" of the volunteer letter for joining the Party, there was written the content of "Resolution to agree to reissue the volunteer letter for joining the Party by comrades", which was signed by the secretary of the Party Committee and stamped with the official seal.

8. The procedures and methods for completing the resolution of preparing party member to become a full member are the same as above. The resolution shall indicate the starting time of the party age, be signed or sealed by the secretary, and be stamped with the official seal of the party organization.

9. The replacement rewrites an application for joining the Party, an application for becoming a full member and a personal autobiography. Fill in the date when you join the party, then fill in the date of replacement and write the word "replacement". The application for joining the Party together with the replacement shall be included in party member's personal file.
