Note:
1. The names of the parties in the notarial certificate should be true. If there are special requirements, they must be carefully examined and verified before being proved. The names of the parties shall be simplified. In foreign language translation, Chinese Pinyin should be used. In order to facilitate the parties to use the notarial certificate, if there is a previous name, it should be proved on the public certificate at the same time after effective proof.
2. The date of birth of the parties in the notarial certificate is one of the core contents of the notarial certificate, which must be accurate, true and have conclusive evidence. The date of birth shall be based on the ID card; if there is no ID card, it shall be based on the household registration certificate or birth certificate; if there is no such certificate, reference shall be made to other certificates (such as files and passports), and the Gregorian calendar shall be adopted. If the notarized parties require otherwise, the date of birth of the lunar calendar may be indicated in brackets.
3. The place of birth of the parties in the notarial certificate should be clear and specific, and the full name of the place name should be written. The address of birth should be written in the city or county, and according to the requirements of the place of use (country), it can also be written in the township, village or town street. If the place name of the party at birth has changed, the full name of the original place name should be stated in brackets after the current place name.
4. The parents of the parties to the birth certificate should indicate the names of the biological parents. If the parents are dead, they should be indicated in brackets. If the parties have been adopted by others, the names of the adoptive parents should be indicated. For the birth certificate of children born out of wedlock, the name of the biological father can be omitted according to the requirements of the parties.
5. If the date of the ID card provided by the party is inconsistent with the date on the overseas certificate (such as passport, pass, etc.), and the change is proposed, if the inconsistency is caused by the work error of the original issuing authority, the party concerned needs to go to the issuing authority to ask for correction, and then give notarization. If the notary lied about his age in order to go abroad at that time, he should be notarized according to his real age after finding out.
6. If a party who has been abroad has applied for a birth certificate, and the notary office has not found any clues after investigation, it can issue a "Notary Certificate for Checking No Files" for the party. In the United States and other countries, this notarial certificate can be used as secondary evidence.
7. The notary office can only handle birth certificates for Chinese citizens or foreigners born in China. People born abroad, if they need such certificates, should obtain birth certificates from relevant local authorities.
