In Chengdu Medical Insurance Bureau, the "Declaration Form for Medical Treatment of Insured Persons of Chengdu Basic Medical Insurance in Different Places" is signed or sealed by the applicant. Retirees need to provide another copy of their household registration, and those with household registration in Chengdu need to provide the original long-term residence certificate or temporary residence permit issued by public security organs in different places. When employees declare to the unit, the unit needs to provide another copy: if it belongs to the overseas branch of the unit, the copy of the organization code certificate or tax registration certificate of the overseas branch must be stamped with the official seal of the unit; If there is no overseas branch code certificate and tax registration certificate, the unit shall provide a written explanation and a copy of the purchase contract, house lease and cabinet lease agreement in different places. If it is a labor dispatch unit, the copy of the dispatch agreement shall be stamped with the official seal of the unit; If the dispatch agreement does not stipulate the place of work, the employer shall issue it in a different place.

In the designated social security medical institutions for medical treatment, the expenses shall be paid in full by myself first. A certificate is issued by the medical insurance bureau at the place where the patient is treated, indicating the level of the hospital and whether it is a designated social security hospital. Within 3 months from the date of discharge (no more than 12 months in special circumstances), the insured shall submit the discharge certificate, expense list, invoice, Chengdu social security card, ID card and my bank passbook to the handling window of the medical insurance bureau for reimbursement by the unit or individual. If it is entrusted to others, a copy of the client's ID card shall also be issued. The medical insurance bureau will review the submitted materials, and notify me to go to the window within 15 working days, and the reimbursement expenses will be directly deposited into the insured's bank account.
