The materials that need to be submitted for the on-site review of the 2016 Physician Qualification Examination are introduced as follows:

(1) Original and copy of my valid identity certificate. Valid identity certificates include personal ID card, military officer ID card, civilian cadre or soldier ID card, mainland travel permit and ID card (for candidates from Taiwan, Hong Kong and Macao), and passport (for foreign candidates).

If candidates have special reasons such as applying for a valid identity certificate when registering, they can submit a certificate issued by the unit where they work, confirmed by the health administrative department at or above the prefectural level, and a recent passport-free photo of themselves to register. Procedures, candidates must present this certificate and other required documents to enter the exam. A copy of the certificate will be kept by the test center for two years.

(2) Original and copy of graduation certificate.

(3) A certificate issued by the candidate’s probation (or internship) institution that the probation (or internship) period has expired for one year and the candidate has passed the examination.

(4) When a practicing assistant physician applies for the practicing physician examination, he or she shall also submit the original and photocopy of the practicing assistant's "Physician Qualification Certificate", "Physician Practice Certificate", practice time and assessment certificate. If the workplace is a medical institution, candidates must submit a copy of the institution's practice license.

(5) Those who apply for the Physician Qualification Examination as a traditional medicine teacher or have expertise must submit a "Traditional Medicine Teacher Certificate" and a "Traditional Medicine Teacher Certificate" and a "Traditional Medicine Teacher Certificate" and "Traditional Medicine Teacher Certificate" Certificate of Qualification Examination for Physician Qualifications of Specialized Personnel”.

(6) Candidate registration photos. The candidate's registration photo should be a 2-inch full-length color photo with a white background taken within the past 6 months (this photo is used for the production of both the "Physician Qualification Certificate" and the "Physician Practice Certificate" and cannot be replaced).

(7) "Notification of Successful Online Registration for Physician Qualification Examination".

Candidates should hold the printed "Notice of Successful Online Registration for Physician Qualification Examination" and written registration materials, etc., and conduct on-site qualification review in accordance with the specific requirements of the examination center. For candidates who are qualified to apply, the examination center will collect proof of identity. Photocopies, original and photocopies of graduation certificates, proof of completion of one-year probation period and passing the examination and other materials. Practicing assistant physicians who apply for the practicing physician examination will also need to collect the original and photocopies of the "Physician Qualification Certificate" and "Physician Practice Certificate" of the practicing assistant. Please print out the "Physician Qualification Examination Registration and Physician Qualification Application Form" on-site. Candidates should carefully check the contents of the printed form and their photo. After verification, they should sign in the prescribed position. The registration information after signature confirmation must not be changed. The test center will issue corresponding documents after collecting the test fee from the candidates.
