1.2 column 1: The name, address and country of the exporter in this column should be the same as the name of the exporter company at the time of registration. The country name and address must be indicated.
For example: Shenzhen Huayelong Import &; China Shenzhen Export Co., Ltd.
Column 2 of 1.3: name, address and country of the consignee. Except EU-27 and Norway, the name of the final consignee in the preference-giving country must be filled in this column, and the name of the intermediate exporter is not allowed. This column does not have to be marked with the names of EU-27 and Norway. If the importing country in column 2 and the final destination country in column 12 are EU countries, the name of 65438 can be used for comparison.

1.4 Column 3: Mode of transportation and route (as far as we know) should generally include loading, arrival place (port of departure and destination) and mode of transportation (such as land, sea and air).

The transshipment port should be added to the transshipment goods, for example, from Shenzhen to Hong Kong by truck and then to Hamburg by sea. 1.5 Column 4: Official use
This column is to be filled in by the visa authority. Normally, this column is blank. Under special circumstances, the issuing authority will add comments in this column, such as: (1) The goods have been exported, and the date of issuance is later than the date of shipment. When issuing the "late issuance" certificate, this column will be stamped with the red seal of "reissue". (2) If the certificate is lost, stolen or damaged, the "duplicate" certificate should be stamped with a red seal, and the serial number and visa date of the original certificate should be indicated in this column, declaring that the original certificate is invalid, and indicating that "this certificate replaces the original certificate number" ... obsolete ... cancelled ".
Note: When issuing the certificate after entry, please indicate "Issued after application" in the remarks column of the application form, otherwise the computer will return it.
1.6 Column 5: Commodity serial number
Due to different kinds of export goods, they are divided into "1", "2" and "3" according to different kinds, and so on. For a single commodity, fill in "1" in this column.
1.7 Case 6: shipping mark and package number
The marks filled in shall be consistent with the marks on the outer packaging of the goods and the invoices; The words made outside China and countries, and the words of origin of Hong Kong, Macau and Taiwan Province Province (such as products from made in taiwan and Hong Kong, etc.). ) shall not appear on the marks and numbers; If the goods are not marked, please fill in "N/M". If there are too many shipping marks and this column is not enough, please mark them (see the attachment), fill in all shipping marks with an attached page (the size of the attached page should be the same as that of the original certificate), mark the certificate number in the upper right corner, and the applicant and the signatory authorized by the visa authority shall sign and seal the attached page in the lower right corner and the lower left corner respectively. The handwriting, location and date of the attached manual are consistent with the columns of the certificate 1 1 and 12.
Note: If there is an attached page, please indicate "marks and numbers sees the attached page" in the remarks column of the application, otherwise the computer will be returned.
1.8 Column 7: The number and type of packages, commodity name and number of packages must be expressed in both English and Arabic numerals.

1) If the number of packages is greater than 1000, there can be no conjunction between 1000 and 100 units, otherwise the computer returns. Fill in: 200 1 forgotten and 500 (2 150) cartoons of word lovers.
2) Quantity and product name should be printed on one page. If the content is too long, you can merge the packing numbers or the product names. Example: A box of gloves, scarves, ties and hats for Hunger and fify( 150).

4) The commodity name must be specified (specific enough to find the corresponding 4-digit HS code), and generally "machine" and "clothing" cannot be specified. Some goods, such as toy fans, should be labeled "Toy: Electric Fan", not just "Electric Fan".
5) The trademark, brand name and article number of the commodity can generally be left blank. After the commodity name and other items are listed, a symbol indicating the end should be added on the next line to prevent forgery. Foreign letters of credit sometimes require the contract number, letter of credit number, etc. , you can add it in the blank.
1.9 Column 8: Standard of Origin
1.9. 1 Completely original products, without any original ingredients, exported to all preference-giving countries, and filled in "P";
1.9.2 For products with non-original ingredients exported to the EU, Norway, Switzerland and Japan, fill in "W" followed by the HS item number of the exported product, for example, "W" 42.02. Conditions: (1) The products are listed in the "processing list" of the above preferential countries and meet their processing conditions; (2) The product is not listed in the "Processing List", but all the non-original raw materials and parts used in the production of the product have been processed, and the HS item number of the product is different from the raw materials and parts used.
1.9.3 For products containing non-native ingredients, fill in "f" when exporting to Canada. Conditions: the value of non-original ingredients shall not exceed 40% of the ex-factory price of products.
1.9.4 For products containing non-native ingredients, enter "Y" for export to Russia, Ukraine, Belarus, Kazakhstan, Czech Republic and Slovakia, and then add the percentage of the value of non-native ingredients in the FOB price of products, for example, "Y" is 38%. Conditions: the value of non-original ingredients shall not exceed 50% of the FOB price of products;
1.9.5 For goods exported to Australia and New Zealand, this column can be left blank.
1. 10 Column 9: Gross weight or other quantity.
Note: This column should fill in the normal units of measurement of commodities, such as "only", "piece", "double", "Taiwan" and "dozen". For example: DOZ 3200. Or 6270 kilograms. If calculated by weight, fill in the gross weight. If it is only a net weight, this net weight is acceptable, but it should be marked with N.W. (net weight).

The final importing country must be filled in the horizontal line of the importing country, and the importing country must be consistent with the port country in the third column.
In addition, the applicant shall authorize a special person to sign this column, indicate the place and date of application, and affix the applicant's Chinese and English seals. The handwriting of the signatory must be registered with the inspection and quarantine bureau and kept relatively stable. The date in this column must not be earlier than the invoice date (10 column) (the earliest date is the same day). When stamping, avoid covering the name of the importing country and the name of the signatory. This certificate shall not be altered or stamped with a proofreading seal.
