The supporting documents include the personal income tax form of the American family (Form 14), the income tax form of the company (Form 12), the quarterly report of the company (Form 941), the W-2 tax form, the bank deposit report or the affidavit of personal financial support (Form I-134), etc. Proof that the applicant has good health insurance in the United States will also be helpful to his application.
