If you forget to sign the health commitment letter, you can hand it in. You can go to the teacher who received the letter of commitment to modify it.

If the commitment letter is handed in without signature, you can go to the relevant teacher to explain the requirements and then modify it.

It doesn't matter much if you don't have a chance to ask the teacher. The letter of commitment generally does not affect the test scores.
