Format of notarial certificate

Notarial certificate consists of three parts: header, body and tail:

(1) Header. Please indicate the title and number, i.e. "xx Notary Certificate", "(xx)x Notary Certificate No. x". Notarial certificates of inheritance, adoption, and kinship should also state the name, gender, date of birth, address and other identity matters of the parties involved.

(2) Text. Also known as testimony, it should state the legal acts or legal facts confirmed by the notary authority based on the matters the parties apply for certification.

(3) Tail. State the full name of the agency that produced the document, and have it signed and stamped by a notary, indicating the year, month and day when the document was issued, and affixing the official seal.
