(2) Enterprises that have applied for online declaration can directly apply for online certification. First, print the voucher. According to the printed voucher, sort out the employee's digital photo receipt that needs to be certified (this receipt is not needed for lost replacement cards) and attach it to the list, and provide a copy of the employee's ID card, submit it to the business card printing window designated by social security, pay the fee and receive the business card printing receipt.
(3) If the online declaration is not made, the enterprise shall submit a copy of the employee's ID card and the receipt of the social security card digital photo (this receipt is not needed for the lost replacement card), and the receipt of the social security card digital photo shall be arranged in the order of the insurance report and attached to the insurance report. If the lost social security card needs to be reissued, you can directly fill in the "X City Social Security Card Replacement (Loss Reporting) Application Form". After the collection department to which the payment belongs prints the voucher preparation list, it can pay at the Shichun business card printing window designated by social security and receive the business card printing receipt.
(4) The person who pays in the individual payment window should submit the original ID card and the digital photo receipt of the social security card (this is not required for those who have already applied for the social security card) to fill in the form, pay the fee and get the printed receipt of the business card.
(5) The insured unit or individual who pays the fee will pick up the card from the original business card printing acceptance window within the specified time with the business card printing receipt.
