1. Copy of student ID card and parent ID card (copy the front of two ID cards on the same A4 paper), mark "Agree to XXX's application for suspension from school" in the blank, sign and indicate the date of writing;

2. If the identity certificate in 1 cannot be provided, a copy of this page of the parents' and students' personal accounts can be provided (copied on the same A4 paper), and the words "Agree to XXX's application for suspension from school" and the date of writing can be indicated in the blank.

3. If neither the student nor the parents can go through the formalities at school, the trustee is required to bring the power of attorney, the trustee's ID card and a copy;

4. A "diagnosis certificate" issued by a hospital above the second level (only students who apply for suspension due to illness need to prepare).
