1. 3. Collect the application form and the cost of work. 4. The bank where the medical insurance account is opened will inform the insured unit to go to Guangzhou Medical Insurance Center to handle the formalities of handing in the form, paying the fee and getting the card after the 19th day of the following month. 5. The agent of the card-issuing insured unit must issue the social security (citizen) card to the insured within one week after receiving the card.
