The procedure for applying for a disability certificate is as follows:
1. Application: Applicants who apply for a disabled person's card for the first time and applicants who apply for a second-generation card need to go to the neighborhood committee to fill out the Application Form for a Disabled Person's Card in People's Republic of China (PRC) (hereinafter referred to as the Application Form) and the Disability Assessment Form in People's Republic of China (PRC) (hereinafter referred to as the Assessment Form) with their ID cards, original household registration books and seven recent two-inch bareheaded photos.
Minors (/kloc-under 0/6 years old) and mentally and intellectually disabled persons must be accompanied by their legal guardians to Jie Zhen Disabled Persons' Federation, and the legal guardians shall apply for it on their behalf, and at the same time provide the guardian's ID card and proof of the relationship between the guardian and the applicant. A voluntary guardian who cannot appoint a guardian shall indicate the specific relationship in the "relationship with him" column of the guardian, fill in the voluntary declaration (voluntary guardian) and sign it.
2. Acceptance: the neighborhood committee checks the applicant, photo, ID card and household registration book (the ID card information of the Ministry of Public Security is extracted through database management software).
3. Disability assessment: The applicant will go to the designated assessment hospital for disability assessment with a photo and an assessment form stamped with the official seal.
4. Preliminary examination: The District Disabled Persons' Federation conducts a preliminary examination of the applicant's relevant materials and the evaluation results of the hospital. If the information is false or the results of disability assessment made by the hospital do not meet the disability standards, they shall be returned. After passing the preliminary examination, the District Disabled Persons' Federation will submit the application form, evaluation form and other materials in triplicate to the Municipal Disabled Persons' Federation for approval.
5. Audit: According to the disability standards and the Measures for the Administration of Disabled Persons' Cards, the auditors of the Municipal Disabled Persons' Federation will audit the applicant's application for accreditation, the acceptance procedures of the District Disabled Persons' Federation, the disability assessment results and the preliminary examination opinions of the District Disabled Persons' Federation. Do not meet the disability standards, the District Disabled Persons' Federation's preliminary opinion is wrong or unclear, and others that do not meet the requirements will be returned and will not be approved. Those who meet the disability standards and handling procedures will be returned to the District Disabled Persons' Federation after signing the audit opinions on the relevant materials, and the Municipal Disabled Persons' Federation will keep the application form, evaluation form and other materials for the record.
6. Print and seal: The District Disabled Persons' Federation prints the disabled person's card audited by the Municipal Disabled Persons' Federation, and keeps the application form, evaluation form and other relevant files. After the official seal of the Disabled Persons' Federation is affixed to the column of the issuing authority, the Disabled Persons' Card will be reported to the Municipal Disabled Persons' Federation. The Municipal Disabled Persons' Federation shall affix its official seal to the column of the licensing examination and approval authority, affix the steel seal to the portrait of the holder and return it to the District Disabled Persons' Federation.
7. Issuance: The applicant brings his/her ID card to the county (city) District Disabled Persons' Federation, receives the second-generation disabled person's card after examination and approval, and returns the first-generation disabled person's card to the street office (township) Disabled Persons' Federation for registration.
8. Archiving: Application form and evaluation form in triplicate. Application form: one for the municipal, district, street and town disabled persons' federations; Evaluation form: the hospital for the disabled, the municipal and district disabled persons' federations each keep one copy for filing. (The white form should be given to the Municipal Disabled Persons' Federation, the yellow form to the District Disabled Persons' Federation, the red application form to the Jie Zhen Disabled Persons' Federation and the red evaluation form to the hospital).
9. Document transfer formalities: Disabled persons apply to Jie Zhen Disabled Persons' Federation with the original household registration transfer certificate. After the Jie Zhen Disabled Persons' Federation issues the certificate, it shall go through the file extraction procedures at the District Disabled Persons' Federation and return the disability certificate (re-apply for the certificate at the new residence). After reporting the loss in the newspaper, you should apply to the Jie Zhen Disabled Persons' Federation with the newspaper information. Jie Zhen Disabled Persons' Federation issued a letter of information on disabled persons, and brought the applicant's ID card and the original residence booklet to the District Disabled Persons' Federation to apply for a replacement.
