Note: 1. If the owner is the unit, fill in this form before the vehicle applies for the inspection mark, and apply for the inspection mark at the security inspection agency with this form;

2. If the owner is an individual, you don't need to fill in this form. After the police enter the annual inspection information of vehicles into the key vehicle supervision system, they apply to the security inspection agency for the inspection mark.

Instructions for completing the form:

1. Motor vehicle owner: to be filled in by the motor vehicle owner registered according to the motor vehicle driving license;

2. Vehicle use nature: according to the actual use nature, it is mainly divided into the following categories: non-operation, road passenger transport, bus passenger transport, tourist passenger transport, taxi passenger transport, freight transport, muck transport vehicle, hazardous chemicals transport, school bus for primary school students, school bus for children and other school buses;

3. Vehicle type and inspection validity period: fill in according to the driving license record;

4. Vehicle violation record: if there is no violation record, fill in "None", if the violation has been handled, fill in "Handled", if the violation has not been handled, fill in "Several Untreated Items";

5. The driver's name, gender, driver's license number, and permitted driving model are consistent with the information recorded in the driver's license;

6. Driving license illegal scoring: fill in the remembered scores according to this scoring cycle;

7. Does the driver have a drunk driving record: fill in the query results on the comprehensive application platform of public security traffic management;

8. Driver's physical health status: fill in according to whether the driver has any health status that hinders safe driving;

9. The enterprise implements the main responsibility: the vehicle unit's safety responsible person compares the vehicle with the driver's situation, and if it meets the above contents, it shall be signed by the safety responsible person and sealed by the unit (vehicles that do not need to be equipped with driving recorder and gps according to regulations shall not be inspected and used);
