1. Students should prepare the original and photocopy of the certificate of Grade-A hospitals (one for each), fill out the application form for exemption from testing in duplicate, sign it and telephone it.

2. Submit the application to Teacher Cai of the Sports Department within the specified time (generally, the form will be collected one week before the test).

3. After the application is approved, go to the website of the Academic Affairs Office to apply for exemption from the test within the specified time, and upload photos of the approved application form.

4. Major diseases or operations can be certified and stamped by the hospital. If knee joint, ankle joint and intervertebral disc protrusion need to be exempted from measurement, CT or MRI report must be provided, otherwise the examination can only be postponed.
