1. When prescribing narcotic drugs and psychotropic drugs of category I for patients for the first time, the doctor with prescription right should personally examine the patients, establish corresponding medical records for them, keep a copy of the patient's identity certificate and ask them to sign an informed consent form. Medical records are kept by medical institutions.
2. Narcotic drug injections are only used in medical institutions, or medical institutions send medical staff to patients' homes for use.
3. Medical institutions should require patients who use narcotic drugs for non-injection and psychotropic drugs of category I to return to the clinic or follow up every 4 months 1 time.
4. When narcotic drugs for non-injection and psychotropic drugs of category I need to be taken out of medical institutions for use, only doctors with prescription rights can prescribe narcotic drugs and psychotropic drugs of category I after the patients or their agents produce the following materials: (1) a diagnosis certificate issued by a hospital above level II; (two) the patient's household registration book, identity card or other relevant certificates; (3) the identity certificate of the agent.
A medical institution shall keep a copy of the identity certificate of the agent in the patient's outpatient medical record.
Three. Requirements for prescription format of narcotic drugs and psychotropic drugs
1. It consists of three parts:
(1) Preface: name of medical institution, prescription number, patient's name, gender, age, ID number, outpatient medical record number, name of handler, gender, age, ID number, subject, date of issuance, etc. , you can add items required by your major.
(2) Text: illness and diagnosis; Mark with Rp or R, and list the name, specification, quantity, usage and dosage of drugs respectively.
(3) Postscript: the doctor's signature, dosage and the signature of pharmaceutical professional technicians who review, allocate, check and distribute drugs.
2. The printing paper of the prescriptions of narcotic drugs and psychotropic drugs of category I is light red, and the top right corner of the prescription is marked with "hemp" and "Jingyi" respectively; The prescription printing paper for psychotropic drugs of category II is white, and the prescription is marked with "Jinger" in the upper right corner.
3. The prescriptions of narcotic drugs and psychotropic drugs are uniformly printed by medical institutions in accordance with the prescribed styles and comprehensively arranged by the medical education network.
Four. Prescription dose requirement
1. Prescriptions for injection of narcotic drugs and psychotropic substances of category I 1 time; The prescription of other dosage forms shall not exceed 3 days; The prescription of controlled release preparation shall not exceed 7 days.
2. The prescription of psychotropic drugs of category II shall generally not exceed 7 days; In some special cases, the prescribed dose may be appropriately extended, but the doctor should indicate the reasons.
3 for cancer pain, chronic moderate and severe non-cancer pain patients with narcotic drugs, psychotropic drugs injection prescription shall not exceed 3 days; The prescription of other dosage forms shall not exceed 7 days.
4. The narcotic drugs that need special control, the dosage of dihydroetorphine hydrochloride is 1 time, and the drugs are only used in hospitals above the second level; The prescription of pethidine hydrochloride is 1 times, and the drug is only used in medical institutions.
5. The prescription of narcotic drugs shall be kept for at least 3 years and the prescription of psychotropic drugs shall be kept for at least 2 years.
