1, signature of immediate family member. When the remains are cremated, the immediate family members of the deceased must sign at the scene. If the deceased has no immediate family members, the collateral relatives will sign on the spot with the certificate of the police station.
2. There must be a formal cremation certificate. If a normal corpse is cremated, a death certificate issued by an approved medical institution, unit or villagers' committee shall be submitted; If the deceased had no fixed unit before his death, the death certificate issued by the town government of his domicile shall be submitted. The local public security department shall issue a death certificate for a corpse that dies abnormally or has no name in a criminal case.
3. ID cards and household registration books of relevant personnel. Including the identity card or household registration certificate of the deceased, the household registration book of the deceased, the identity card of the relative who sent the corpse or the valid certificate that can prove the relationship with the deceased.
Second, the materials needed to issue a death certificate
Go to the police station to issue a death certificate. Immediate family members can bring the deceased's ID card and household registration book to the police station to issue a death certificate. If a citizen dies abnormally or the health department cannot determine whether he died normally, the public security and judicial departments shall issue a death certificate; If a citizen dies in a medical and health unit, the hospital shall issue a medical certificate of death. If a citizen dies normally and cannot obtain the death certificate issued by the hospital, the certificate issued by the neighborhood (village) committee or the health station (institute) shall be used.
According to the Law of People's Republic of China (PRC) on Medical Practitioners and the Regulations on the Administration of Medical Institutions, when issuing the Medical Certificate of Death, the hospital is only responsible for issuing the Medical Certificate of Death to the resident who died in the process of diagnosis and treatment in this unit, and the following procedures must be strictly implemented: 1, according to the national hospital work regulations and hospital work system, the hospital will issue a certificate to the patient's family in time after the patient dies. 2, by the attending doctor (medical practitioners) to fill in the "death report card".
