1. Applicants must fill in the form carefully and truthfully according to the content. The Provincial Construction Bureau will not accept any application that is not standardized, has illegible handwriting, or has incomplete seals.

2. When filling in this form (except for signature and seal), you must use a computer to print it. If the number of columns (number of pages) in the application form is insufficient, you can add more, but the format must be consistent.

3. For the personnel category, fill in the main person in charge of the enterprise or the person in charge of the project or the full-time safety production management personnel.
