Preparation materials:

1. application form for qualification of engineering survey and engineering design enterprises (filled in and printed in the national survey and design industry qualification management information system, with two copies and one electronic document);

2. A copy of the business photo of the enterprise as a legal person;

3, the original copy of the engineering survey qualification certificate;

4, engineering survey, engineering design qualification application form listed in the technical personnel title certificate, graduation certificate, ID card copy and technical backbone basic situation and performance table;

5. After the implementation of the relevant practice registration system, the state shall also provide copies of the ID cards and registration certificates of the registered practitioners listed in the Application Form for Qualification of Engineering Survey and Engineering Design Enterprises, or copies of the practice qualification certificates stamped with the practice seal (initial registration), the persons applying for registration change, and the registration change form stamped by the original registration unit, the applicant for qualification and the original provincial registration management department. The initial registrant shall also provide an initial registration form stamped by the provincial registration administration department where the enterprise is registered;

6. A copy of the social security certificate of the last month listed in the labor contract signed by the technical personnel and the enterprise according to law and the application for engineering survey and design qualification. Among them, the main professional and technical personnel engaged in engineering design compilation in the army or colleges and universities do not need to provide social security certificates, but they need to provide personnel certification materials from the superior personnel department of their units. Enterprises applying for increasing the qualification of engineering survey shall provide it in accordance with the provisions of Article 13 of the Regulations on the Administration of Construction Engineering Survey and Design Qualification.

Note: (1) Attachment materials are bound on A4 paper. The attachment materials shall list the detailed contents and page numbers in the order of the above application materials, and have the catalogue and classification number; (two) the applicant shall truthfully fill in the "application form for the qualification of engineering survey and engineering design enterprises", and the legal representative of the enterprise shall sign the application form and be responsible for its authenticity; (three) the application materials should be clear and complete, incomplete information, illegible handwriting and unclear seal, and the qualification acceptance department shall not accept it; (four) the above application materials are required to submit a copy and verify the original. The copy shall be stamped with the official seal of the enterprise, and the copy of the Registration Certificate of Registered Practitioners shall be stamped with the fresh seal of individual practice. Otherwise, the copy is invalid.
