1. Bring your ID card to the local police station to fill in the Registration Form of Lost Household Register and the Registration Form of Lost Household Register (all adult members of this household must sign the application);

2. If the householder cannot apply to the police station for other reasons, he shall issue a written entrustment application and sign it.

Legal basis: Article 17 of the Regulations on Household Registration: If the contents of household registration need to be changed or corrected, the head of household or himself shall report to the household registration authority; If it is verified through examination, the household registration authority shall change or correct it.

When the household registration authority deems it necessary, it may require the applicant to issue a certificate of change or correction.
