Matters needing attention in applying for re-birth approval form

1. The column of "photos of husband and wife" shall not force the applicant to provide photos of parents and children.

5. If the documents provided by the parties are incomplete, the accepting unit shall inform them of the supporting materials that need to be supplemented at one time.

6. The village, township and county levels shall clearly state the provisions of laws and regulations and the original text of the basis for agreeing to give birth again in the opinion column, and the auditor shall sign and affix the official seal. If the decision is made through collective discussion, it shall be signed and sealed by the principal responsible person.

7. After the parties fill in the Approval Form for Re-birth Application and report it to the unit or village (neighborhood), the unit or village (neighborhood) directly reports it to the people's government of a township (town) and the street office, and the people's government of a township (town) and the street office directly reports it to the county-level population and family planning department after the audit, and the applicant shall not be allowed to handle it by the Approval Form for Re-birth Application during the submission process.
