Declare the original and photocopy of the ID card, education background, degree certificate, professional examination certificate and computer application level (ability) examination certificate of intermediate personnel 1 copy. The original is returned on the spot after examination and bound in the upper left corner as required.

3. The applicant recently bareheaded with six original photos (five one-inch photos are posted at the designated position of the declaration form, and 1 two-inch certificate is reserved), and the color and black and white are not limited.

4. Submit papers in quintuplicate at the intermediate level.

5.
