Who should sign the operation record?
Surgical records should be written and signed by the operator in principle, and only by the first assistant under special circumstances. It is absolutely forbidden to write or sign by doctors who are not involved in the operation. According to Article 22 of the Basic Specification for Medical Record Writing, the operation record refers to the special record written by the operator to reflect the general situation of the operation, the operation process, the findings and handling during the operation, and should be completed within 24 hours after the operation. Under special circumstances, when written by the first assistant, it should be signed by the operator. Item 15 of Article 22 of the Basic Code for Medical Record Writing refers to the continuous record of the patient's condition and diagnosis and treatment process after the admission record.
