(1) Collection Form

Applicants can go to the Exit-Entry Administration Office of Wuhan Public Security Bureau or Jiangnan Acceptance Point (Science and Technology Exhibition Center of Donghu New Technology Development Zone 1 Building) to get the Pass for Mainland Residents to Travel to and from Hong Kong and Macao, or download the form directly from the Internet (website: www.whcrj.gov.cn) (printed on A4 format paper). Residents of Jiang 'an District, Jianghan District, Qiaokou District, Wuchang District, Qingshan District, Wuhan Economic and Technological Development Zone, caidian district, Jiangxia District, huangpi district District and Xinzhou District can also get the form from the public security bureau where their household registration is located.

(2) filing an application

Residents of Jiang 'an, Jianghan, Qiaokou, Wuchang, Qingshan and Wuhan Economic and Technological Development Zones may apply to the branches where their household registration is located.

Residents in Caidian, Jiangxia, Huangpi and Xinzhou can only go to the branches where their household registration is located; The employees of Changhang, Wu Tie and Civil Aviation can only be accepted by the exit and entry administration departments of Civil Aviation, Changhang and Railway Public Security Bureau.

The Exit-Entry Administration Office of the Municipal Bureau or Jiangnan Acceptance Center (Science and Technology Exhibition Center of East Lake New Technology Development Zone/Building KLOC-0/) can accept the applications of urban residents in the whole city.

The following materials shall be submitted when applying:

1. Submit and fill in the complete Application Form for Mainland Residents' Travel Permit to and from Hong Kong and Macao.

2. Submit digital photos. In order to ensure the quality and size of photos, the applicant himself can take digital photos at the accepting institution or five photo spots in our city (see the attached table for the name and location).

3. The national staff shall submit the opinions of the competent department (see Article 2 of the Instruction for the scope of national staff).

4. Submit the original valid resident ID card and household registration book and submit a copy (the applicant is under the age of 16, and must submit my resident registration book and one parent's resident ID card; If parents cannot accompany them, they need to submit a power of attorney and the identity certificate of the trustee; Soldiers only need to submit identification); China citizens who have settled abroad should submit a valid China passport and proof of settlement abroad, and submit a copy; Holders of valid passes must also submit a pass to and from Hong Kong and Macao. If the certificate is lost, a written certificate loss statement is required.

5. The exit and entry administration department of the public security organ only accepts the application for entry and exit permits and endorsements submitted by me, except for the following circumstances:

(1) The applicant is under the age of 16 or has reached the age of 60, and can entrust others to apply on his behalf.

(2) In the same accepting department, if the holder of the same travel permit to and from Hong Kong and Macao applies for endorsement again for the same reason, he may entrust others to apply for endorsement on his behalf, but it does not include the application for business endorsement in different places.

(3) If an applicant who goes to Hong Kong and Macao for employment through a labor management company applies for a pass and endorsement to and from Hong Kong and Macao again during his employment in Hong Kong and Macao, the labor management company designated by the Ministry of Commerce shall apply.

(4) Mainland residents who hold stay endorsements in Hong Kong or Macau can apply through China travel agencies in Hong Kong or Macau.

In the above case, the client must submit the power of attorney, the original identity certificate of himself and the client, and a copy. The labor management company shall put the company seal and the signature of the person in charge in the entry-exit management office of Wuhan Public Security Bureau for the record in advance.

6. Submit the certificate corresponding to the reason for going to Hong Kong and Macao (see Paragraph 3 of Article 2 for details).

(three) to receive the application receipt and pay the certificate fee.

When the applicant receives the application receipt, he should carefully check and sign for confirmation. After receiving the application receipt, you can go to the General Payment Book of Non-tax Revenue in Hubei Province issued by the billing office with the receipt, pay the certification fee at the commercial bank, and return the application receipt and payment receipt exchanged by the billing office.

(4) obtain a certificate.

I hold my ID card, application receipt and payment receipt, and collect them at the time indicated in the receipt. If I can't come to get the certificate, I can entrust my relatives to get the certificate with my ID card, application receipt, payment receipt and proof (such as household registration book, birth certificate, marriage certificate, notarial certificate, etc.) that can prove the kinship. ). Applicants can also choose to handle postal express delivery voluntarily. If the application receipt is lost, I must take it with my resident ID card.
