1, the first item has two situations:

(1) If the insured is under the age of 18 (underage), then the applicant must fill in his guardian (provide four copies of his guardian's passbook);

(2) If the insured is over 18 years old (i.e. adult), then the insured must fill it out by himself (provide copies of the bankbooks of the four major banks);

2. Item 2:

Fill in the insurer's information and the time, place and reason of the accident; (for example, on a certain day, in a certain place, due to a fall, etc. )

3. Item 3:

Fill in the insurance information (true and correct);

4. Item 4:

Explain the process and result of the accident in concise words; (For example, on a certain day in a certain year, somewhere, I fell on the roadbed when I was riding a bicycle and turned, resulting in a fracture of my right hand. I went to the hospital for treatment, and I'm fine now. )

5. Item 5:

Signature of the applicant.
