The Chinese pinyin of the surname, taking "李思" as an example, this line should be filled with LI. (Exactly the same as the passport)
Second line: Name
The Chinese pinyin of the name, taking "李思" as an example, this line should be filled in SI. (It must be completely consistent with the passport)
The third line: gender, date of birth
Just tick the corresponding boxes for gender.
For the date of birth, fill in the numbers in the order of date, month, and year. For example, if you were born on September 1, 1996, fill in 01/09/1996.
The fourth line: town and country of birth
Write Chinese pinyin or the corresponding English name, such as BEIJING, HONGKONG, and fill in CHINA for the country.
Line 5: Nationality and occupation
For international, fill in CHINA.
Occupations, such as: STUDENT, SELF-EMPLOYED, TEACHER, MANAGER, etc.
Line 6: Contact address in the UK
This can be filled in with the complete and detailed address of your dormitory, hotel or relatives. Generally fill in the order of room number, hotel address name, street name, city or town name, and zip code.
