Fill in personal information according to the question guide, and improve the residential address and household registration (the residential address should be detailed to the house number, and the household registration address should be consistent with the household registration book or ID card address. Read the Notice of Residents Signing Family Doctors' Health Management Obligations and Dongguan Family Doctors' Signing Service Agreement, and enter the signature confirmation link. Wait patiently for the review by the family doctor (complete the review within 72 hours).
