How to fill in the China Southern Airlines Travel Health Declaration Form online:

1. Open and log in to the WeChat APP on your mobile phone.

2. On the WeChat main interface, search for the airport line and click to enter the official account.

3. On the airport travel interface, click My - Health Declaration on the lower right menu.

4. To fill in the Airport Passenger Health Declaration, enter the flight number and departure date, and click on the Airport Passenger Health Declaration to fill in the form.

5. In the application filling interface, enter the signature, name, ID, mobile phone number and health status of the applicant, and check the box to read the agreement.
