2. Application conditions

(1) Registered within the administrative region of Sichuan Province, a central enterprise in Sichuan, and has obtained approval from the construction administrative department Enterprises that have issued construction enterprise qualification certificates or engineering design and construction qualification certificates;

(2) Meet the conditions listed in the "Regulations on the Administration of Safety Production Licenses for Construction Enterprises" (Ministry of Construction Order No. 128) .

3. Application Materials

(1) First-time application for production safety license:

1. "Application Form for Production Safety License for Construction Enterprises" (in Fill in and print out two copies in the "Safety Production License Management System";

2. Attachments (one set):

①Enterprise legal person business license, qualification certificate, Organization code certificate;

② The production safety responsibility system at all levels of the enterprise, production safety rules and regulations, operating procedures directory and content (see attachment 1 for details);

③ The enterprise ensures safety Documents proving the capital investment in production (see attachment 1 for details);

④ Documents that the enterprise has set up a production safety management organization and equipped with full-time production safety management personnel (see attachment 1 for details);

⑥ List of company special operations personnel and operating qualification certificates (See Attachment 3 for details);

⑦ Annual safety training plan and education status of enterprise managers and operators (See Attachment 1 for details);

(2) Application for renewal of production safety license:

The validity period of the production safety license is 3 years. Construction enterprises whose safety production license expires and needs to be extended should apply to the Department of Housing and Urban-Rural Development for extension procedures three months before expiration.

1. Enterprises that strictly abide by laws, regulations, rules and mandatory engineering construction standards related to production safety during the validity period of the production safety license, and have no records of bad behavior in the integrity files verified by the Department of Housing and Urban-Rural Development , you only need to provide the following application materials for verification:

① "Application Form for Extension of Safety Production License" (fill in and print out in duplicate in the "Safety Production License Management System");

< p> ②Enterprise business license, qualification certificate, original safety production license, enterprise "three categories of personnel" safety production assessment certificate, special operations personnel operation qualification certificate ("three categories of personnel" safety production assessment certificate and special operations personnel operation qualification certificate A copy of the qualification certificate (which should be valid before the enterprise's production safety license is approved for extension) (the original must be submitted for verification);

③ The Department of Housing and Urban-Rural Development deems it necessary to provide other application materials that further explain ①-②.

2. When a construction enterprise that falls within one of the following scopes applies for an extension, it shall be reviewed according to the conditions for a new application for a production safety license. The enterprise shall submit materials in accordance with the requirements for the first application for a production safety license:

< p> ① During the validity period of the production safety license, a production safety accident occurred and he was responsible for the accident;

② During the validity period of the safety production license, the safety production license was temporarily withheld;

③ During the validity period of the work safety license, the company has been subject to administrative penalties and notices of criticism by the construction administrative departments at all levels more than 3 times (including 3 times) or the safety production dynamic management evaluation score is less than 70 points (inclusive) 70 points);

④ The validity period of the production safety license has expired and no extension application has been submitted;

⑤ Re-examination is considered necessary for other reasons.

If the enterprise falls under any of the circumstances ①, ②, or ③ of this paragraph, the enterprise shall re-apply for a work safety license. After the preliminary review and public announcement by the Department of Housing and Urban-Rural Development, it will be forwarded to the Provincial Construction Project Quality and Safety Supervision Center to organize the company's construction site safety inspection. The production conditions will be verified and the verification conclusion will be reported to the Department of Housing and Urban-Rural Development within 7 working days.

4. Other requirements

(1) The "Safety Production License Application Form" should be printed on A4 computer paper (log on to the website of the Sichuan Provincial Department of Housing and Urban-Rural Development at www.scjst.gov. cn "Safety Production License Management System", after completing the relevant information and reporting it, directly print out the application form with the bar identification code);

(2) Applicants must truthfully fill in the "Safety Production License Application" Form", the legal representative of the enterprise must sign on the application form and be responsible for its authenticity;

(3) The application attachment materials should be bound into a volume on A4 paper and should be listed in the order of the application materials listed above. Detailed catalog and page numbers, as well as catalog and classification numbers (the binding order should be consistent with the order of the catalog, and the order of materials from relevant personnel of the enterprise should be consistent with the order in the production safety license application form);

(4) Application materials It must be clear and complete. If the data is incomplete, the handwriting is illegible, the seal is unclear, or it is difficult to read, the qualification acceptance department may not accept the application;

(5) Copies of all application attachment materials must be submitted, and the originals must be verified. The copy should be stamped with the official seal of the enterprise, and the copy of the registration certificate of the registered practitioner should be stamped with the personal seal of the practitioner. Otherwise the copy will be invalid. (The copy must be the same size as the original. The copy of the certificate should include the number page, photo page, content page, and if there is a renewal record or change record, it should also include copies of the renewal page and change page).

5. Application Procedures

(1) Applicants must first log in to the "Sichuan Provincial Department of Housing and Urban-Rural Development" portal (www.scjst.gov.cn) and enter the "Electronic "Government Affairs Hall" fills in the "Safety Production License Application Form" in the "Safety Production License System" and prints it;

(2) The applicant shall submit it to the construction administrative department of the people's government at or above the county level where the enterprise's industrial and commercial registration is located Make an application and submit relevant materials. (3) The construction administrative department of the city, state or power expansion pilot county (city) should within 20 working days After the preliminary review of the application materials is completed, sign the preliminary review opinions on the qualification application form and submit the preliminary review opinions, official submission letter and all application materials of the enterprise (which must be stamped with the seal of the preliminary review department) to the window of the Housing and Urban-Rural Development Department of the Provincial Government Service Center;

If the application materials are submitted by the enterprise itself, they should be sealed by the preliminary review department.

(4) The Department of Housing and Urban-Rural Development will conduct the review, and the review opinions will be posted on the website of the Department of Housing and Urban-Rural Development (www.scjst.gov.cn) and the website of the Provincial Government Service Center (www.sczw) within the promised time limit. .gov.cn);
